Summit Overview

The PLP Summit celebrates the impact of 15 years of partnership and collaboration across the care continuum to implement improvement initiatives in complex adaptive health systems. The Summit will explore how to leverage a supportive team-based culture using clinical information to drive improvement and be a positive influence as part of the bigger health system evolution.

Keynote Speaker

Victor M. Montori, MD is the Robert H. and Susan M. Rewoldt Professor of Medicine at Mayo Clinic. An endocrinologist, health services researcher, and care activist, Dr. Montori is the author of more than 750 peer-reviewed publications and is among the most cited researchers in clinical medicine and in social science. He is a recognized expert in evidence-based medicine, shared decision making, and minimally disruptive medicine. He works in Rochester, Minnesota, at Mayo Clinic’s KER Unit, to advance person-centered care for patients with diabetes and other chronic conditions. He is the author of the book Why We Revolt, and is leading a movement, a Patient Revolution, for Careful and Kind Care for all. 

Summit Objectives

Enhance patient care through:

1. Fostering partnerships and encouraging ongoing collaborations with key stakeholders, including physicians, healthcare professionals, patients, community partners, and administrators, for understanding problems and co-creating sustainable improvement solutions

2. Highlighting improvement methodologies and their impact on the healthcare landscape

3. Illustrating how user-focused approaches contribute to the successful integration of clinical information and evidence into practice

This webinar series held over Zoom will examine symptom presentations for common neurologic conditions to assist healthcare providers in:

  • Determining which neurologic symptoms are more worrisome and may indicate serious progressive disease
  • Selecting an effective referral strategy to expedite diagnosis of serious neurologic conditions
  • Distinguishing between common and more serious progressive diseases in order to come up with a differential diagnosis

HEARTS symposia for World Hypertension Day 2024

HEARTS Program - A Primary Care Initiative for Canada

The World Health Organization (WHO) developed HEARTS, a state of the art highly systematic primary care intervention to control hypertension, the leading reversible global risk for death. HEARTS has more recently expanded to include diabetes control. HEARTS provides more effective and less expensive care and is designed to strengthen primary care.

We invite you to join us for this webinar on HEARTS that includes representatives from the WHO, PAHO, Kaiser Permanente, and the Crowfoot Primary Care Network. This webinar will demonstrate how the HEARTS program can be implemented in Canada to enhance hypertension control and reduce cardiovascular disease, while strengthening primary care and reducing costs.

A courage-building leadership program

The Program

Based on the research of Dr. Brené Brown, Dare to Lead™ is an empirically based courage-building program designed to be facilitated by organizational development professionals.

The most significant finding from Brené’s latest research is that courage is a collection of four skill sets that are teachable, measurable, and observable. The Dare to Lead™ program focuses on developing these courage building skills through workshops, trainings, and coaching to help individuals, teams, and organizations move from armoured leadership to daring leadership.

Individuals who successfully complete the full 24-hour Dare to Lead™ program will receive a certificate of completion and are allowed to put the Dare to Lead Trained badge on their LinkedIn account.
